Payment Information

 

Fees. Please contact me for rates.

Cash, checks, and Venmo are accepted forms of payment and fee should be paid at time of visit. 

 

Insurance. I am an out-of-network insurance provider. Insurance plans tend to offer some level of reimbursement, but please check with your insurance company prior to our first meeting for specifics regarding your plan and coverage. For those clients with out-of-network insurance coverage, you should expect to pay me agreed upon fees directly and then seek reimbursement from the insurance company on the basis of my bill.

 

Cancellation Policy. If you have to change or cancel an appointment with less than 24 hours notice, you will still be responsible for the fee for that session. Please call directly to change or cancel appointment. If you text, do not assume the appointment is cancelled until I have confirmed receipt of your text.

 

Forms
 

Intake Packet. Prior to your initial appointment, please download and fill out the intake packet and Telehealth Consent form, and return to me before our first session together.
